- The Greater St Louis Construction Laborers Welfare Fund is required by law to maintain the privacy of your health information and give you notice of our legal duties and privacy practices regarding your health information. The notice of Privacy Practices explains how the plan may use and disclose your health information. The notice is printed in your Summary Plan Description at pages 19:1 to 19:6. You may also request a paper copy of the notice from the Fund office by sending a request to the fund.
- If you have Other Insurance Coverage that is primary for prescriptions and wish to be reimbursed for your co-pay please call the fund office for a form to send directly to MEDIMPACT. (Do not send to Healthlink).
- DME (Durable Medical Equipment) needs to be pre-approved. Please call the fund office to get approval.
